Sagliker Syndrome – A Case Report

Aim: To present a case of Sagliker syndrome (SS), rare caused by longterm heavy tertiary hyperparathyroidism (HPT) in end-stage renal disease (ESRD). Case report: In the year 2000, 38-year-old man was diagnosed with ESRD. He is currently low height, paraplegic, pigeon and barrel chested, elongated upper extremities, deformed fingers, mandibular maxillary asymmetric deformities teeth malformations. Due to extremely high serum parathormone (PTH), resistant pharmacological treatment, subtotal parathyroidectomy (PTx) performed 2007. However, he underwent parathyroid resurgery 2010 for persistently PTH more than 2500 pg/mL (upper normal limit 69). 2012, imaging found two suspected glands one them surgically removed. Postoperatively, expected decrease calcium concentrations did not occur. Ten years after diagnosis ESRD, patient began notice pronounced skeletal (upper, lower jaw, kyphoscoliosis) along depressive disorder. Laboratory findings still show (1994 pg/mL), calcium, 1.89 mmol/L (normal range 2.14–2.53), alkaline phosphatase, 837 U/L 60–120), despite continuous treatment. Conclusion: SS firstly recognized Yahya 2004 it has been described Croatia date. Pervasiveness knowledge poor. The most efficient way treating/preventing early total PTx severe secondary HPT related can only stop progress disease, but cannot return deformities.
